Six steps to success
At Dignus we have developed a robust six step process for managing the successful transition and placement of any individual.
Step One: Assessment.
A number of individual ‘needs assessments’ are undertaken to determine the specific requirements and to identify the most appropriate support plan. These assessments involve the individual, relatives and social services or health authority care managers, in addition to the specific home managers and support teams.
Step two: Placements Options.
Our experienced placement team, working closely with the service manager’s work to identify the services that would suit the individual’s assessed needs.
Step Three: Approvals.
The service manager then liaises with the funding panel to obtain approval for the placement and will be responsible for keeping everyone informed on the progress of the application.
Step Four: Transition
Once a placement is accepted, a tailored transition is arranged, which may include meals and overnight stays at the preferred service to ensure the individual is comfortable and familiar with their new home.
Step Five: Support.
During the transition process, a plan is developed for the individual to ensure that they are receiving the appropriate support to meet all of their assessed needs, including life-skills, community interaction, hobbies and interests. In addition to this education and training pathways are developed where appropriate.
Step Six: Review
Ongoing reviews take place throughout the placement to ensure that the individual’s needs and goals are being met. These are in addition to the statutory reviews required under all regulatory regimes.
